Grand Rapids High School Football · Parental Permission
This form gives permission for your son to take part in the DēsAL™ program, a health and wellness program run with the Grand Rapids High School football program in partnership with Northern Arizona University.
Each week players receive brief coaching content and complete a short check-in. Periodically we do a quick fingertip scan, about 15 seconds and completely non-invasive, that uses light to estimate carotenoid pigment stored in the skin. It is not a medical test and it does not diagnose anything.
Participation is voluntary. Your son can stop at any time with no impact on his standing on the team.
Program: DesAL GRHS Program. Operational reminders only: scan day reminders, weekly check-in reminders, and program assessment reminders.
Texting is optional. You can enroll your son fully without agreeing to any text messages.
Message frequency varies, up to 5 messages per week.
